Oxycodone vs Hydrocodone — Which Is Right For Your Pain?

A comparison of two of the most commonly prescribed opioid pain medications.

Overview

Oxycodone and hydrocodone are both semi-synthetic opioid analgesics prescribed for moderate to severe pain. While they share a similar mechanism of action, they differ in potency, common formulations, and typical use cases. The right choice always depends on your prescriber's evaluation of your specific condition.

Potency and Onset

Oxycodone is generally considered somewhat more potent than hydrocodone on a milligram-for-milligram basis. Both have a relatively fast onset when taken orally, with immediate-release formulations beginning to relieve pain within 15–30 minutes, and extended-release formulations providing longer, steadier coverage over 12 hours.

Common Formulations

Oxycodone is available alone (e.g., Roxicodone, OxyContin) or combined with acetaminophen (Percocet). Hydrocodone is most commonly combined with acetaminophen (Norco, Vicodin). Combination products carry acetaminophen dosing limits that your prescriber will account for.

Side Effects

Both medications share a similar side effect profile, including drowsiness, constipation, nausea, and risk of dependence with prolonged use. Your prescriber will monitor your response and adjust treatment as needed.

Which One Is Right for You?

Only a licensed prescriber can determine which medication and dosage is appropriate for your condition, medical history, and pain severity. Never adjust dosing or switch medications without medical guidance.
